This form is designed to help you compile a clear and complete summary of your financial situation. California law requires both parties to a premarital agreement to provide full and honest disclosure of their income, assets, debts, and expenses.
The information you provide will be used to prepare your official financial disclosures and to support the drafting or review of your premarital agreement. Please complete each section to the best of your ability. Our team will review and finalize the disclosures with you.
This is an internal working document between you and our firm. Once finalized, we will prepare a formal disclosure from this information for your signature and exchange with the other party.
Instructions
Complete each section as thoroughly as possible. If a particular subsection does not apply (e.g., you have no cryptocurrency or student loans), click the "N/A — skip" button next to the subsection heading to collapse it.
List every open account — even those with a zero or minimal balance. This includes credit cards you rarely use, dormant savings accounts, and any open line of credit. If it is open, it must be disclosed.
For accounts and debts, provide the institution name, account type, last four digits of the account number, and the current balance as shown on your most recent statement.
Amounts should match your supporting documents wherever a statement exists (bank accounts, credit cards, loans, retirement accounts, investment accounts, income). Estimates are acceptable only where no statement is available — such as real property values (use Zillow), vehicle values (use KBB), and personal property.
